Purpose:


These terms and conditions are  designed to ensure students know their rights and responsibilities regarding the UFVSUS U-Pass/Campus Shuttle opt-out process and protect UFVSUS staff from assuming liability in the case of an unsuccessful or unreceived request.

Opt-out requirements are stipulated in the contractual agreement between UFVSUS and BC Transit and are reiterated here to assist in the opt-out process. 

Terms and Conditions: 

  1. This form was filled out accurately and honestly. 

  1. I meet (at least) one of the three opt-out requirements as outlined by UFVSUS and BC Transit. 

  1. I have read the "Required Criteria for Courses To Be Considered Fully Online" section of the U-Pass or Campus Shuttle webpages on the UFVSUS website.

  1. I can confirm that I have a registered disability with the UFV Centre for Accessibility Services.

  1. I confirm that my BC Bus Pass matches the image of the example card displayed under the "Opt-Out Requirements" sections of the U-Pass and Campus Shuttle webpages on the UFVSUS website. 

    1. Please note: The only form of BC Bus Pass approved for opt-outs are those issued by BC Transit to individuals designated as low-income seniors or as a  person with a disability 

***Students are only agreeing to the stipulation above that coincides with their reason(s) for opting out of the U-Pass and/or Campus Shuttle.

  1. I agree that all supporting documents must be submitted before the conclusion of the active opt-out period, or my opt-out request will be denied..

  1. I am responsible for saving proof of the opt-out form submission confirmation page and certify that I will keep documentation of this page until my opt-out status is confirmed. 

  1. Failure to provide proof of submission upon request by UFVSUS may result in denial of my opt-out request.

  1. I understand that if my request is submitted outside of an active opt-out period, it will not be processed by UFVSUS.
