Purpose:

These terms and conditions are designed to ensure students know their rights and responsibilities regarding the UFVSUS U-Pass Opt-In process and protect UFVSUS staff from assuming liability in the case of an unsuccessful or unreceived request.

Opt-In requirements are stipulated in the contractual agreement between UFVSUS and BC Transit and are reiterated here to assist in the opt-in process. 

Terms and Conditions: 

1. This form was filled out accurately and honestly.

2. I meet all of the three opt-in requirements as outlined by UFVSUS and BC Transit. 

    2.1.  I can confirm that I was registered in courses for Winter 2026.

    2.2.  I can confirm that I have been assessed and have paid SUS fees, including U-Pass fees.

    2.3.  I confirm that I agree to be assessed fees to proceed with opting in to the Summer 2026 U-Pass Program.

         2.1.3. Please note: This will include the U-Pass fee of $44.26 and a  $5.00 SUS opt-in administration fee. 

   *** Students are only agreeing to the stipulation above that coincides with their reason(s) for opting in of the U-Pass for Summer shuttle. 

3. I agree that all supporting documents must be submitted before the conclusion of the active opt-in period, or my opt-in request will be denied.

4. I am responsible for saving proof of the opt-in form submission confirmation page and certifying that I will keep documentation of this page until my opt-in status is confirmed.

   4.1. Failure to provide proof of submission upon request by UFVSUS may result in denial of my opt-in request..

5. I understand that if my request is submitted outside of an active opt-in period, it will not be processed by UFVSUS.
